HOME BIRTHS

Parents who birth their baby at home or outside a birthing facility, will register the birth of their baby with the local vital statistics office.

To register the birth parents are required to provide evidence of pregnancy, evidence of birth and the mother’s residence in Ohio at the time of birth.  The Checklist for Registration of an Out of Institution Birth is a handy checklist which provides information on the requirements that must be presented in order to register the birth.  The details regarding the birth including the name of the baby are provided on the Home Birth Worksheet.   This worksheet is required to generate the birth record.
