Applications for a permit to install a
private water system on your new or existing property are in
duplicate and can be filled out at our office or can be
mailed. For the correct forms, please contact the Delaware
General Health District at 740-203-2070.
The process for approval of the permit is as
follows:
1. All of the application forms must be
completed, by either the homeowner or a registered Private
Water System contractor.
2. Submit the completed Private Water Systems
Application with the application fee to the Delaware General
Health District. Once received, a Sanitarian will conduct
a site inspection to ensure the requirements of OAC 3701-28
are being met. If all rule requirements are satisfied, the
sanitarian will approve the permit. Once approved, payment
will be processed. The permit expires one year from the
date of approval.
3. Often, the Private Water Systems
contractor will take care of the permit paperwork for the
homeowner. If this is not the case, the person who obtains
the permit should provide a copy of the processed
application to the Private Water Systems Contractor that
will do the work.. The processed
application will have a permit number in the upper right
hand corner and an audit number on the bottom right hand
corner.
4. The Contractor will install the
Private Water System as per Ohio Administrative Code.
5. The Contractor(s) must thoroughly complete
a well log and pump completion form and submit a copy of
each to the Delaware General Health District. If the pump is
installed by the homeowner, then the homeowner is
responsible for submitting the pump completion form.
6. The homeowner or representative must call
the Delaware General Health District to schedule a water
sample. Water Samples are only scheduled on Tuesdays and
Thursdays after 12pm.
Please note: A satisfactory water sample
MUST be obtained within one year of completion of the
system. If no water sample is obtained, the system will be
disapproved.
7. If the installation meets OHIO
ADMINISTRATIVE CODE 3701-28, all the paperwork is
acceptable, and the water sample is negative for total
coliform and the nitrate level is less than ten (10) parts
per million (ppm), then the private water system will be
approved.
