Paternity Affidavits

A paternity affidavit establishes fatherhood for children whose parents are not married.  Most couples file this affidavit in the hospital before discharge.  For those couples who do not complete the affidavit in the hospital the Registrar’s office is prepared to assist.  To complete the affidavit both parents of the child must each complete a section and have it notarized. There is no charge for this service.
