births that have occurred in Delaware County. Some date back to 1867.
The
cost for certificates
is $16.50 for each certified copy.
Additional costs apply if ordering birth certificate online.

Many ways to get a
birth certificate:
Order by Phone
- Call 1-877-442-3787.A customer service representative from USCerts
will take your request over the phone, complete payment with a credit
card and process your order. Upon notification of your order we will
prepare and then send the certificates out to you within 24 hours via
United States Postal Service unless you select expedited service of
overnight or second day mail.
Order Online
- Go to
www.USCerts.com to apply for a birth certificate. Complete the
online application form. Submit your payment information and your
order will be prepared and sent within 24 hours via the United States
Postal Service!
Walk in Service
–
office hours are 8:00 – 5:00. Come to
second floor of 1 West Winter to the Vital Statistics Office. In most
cases orders are processed with a short wait.
Mail Order
– Complete the certificate
request form available on this site. Mail request form, with
proper payment of personal check (Ohio only), or Mail order payable to
the Delaware General Health District to:
