Hep A 1st dose at 12 months and second dose 6-12 months later
15-18 Months DTaP DTaP is given at least 6 months after the 3rd dose.
4-6 Years Var#2, DTaP, IPV, MMR#2, Hep B At or before school entry.
11-12 Years Var., Td/Tdep, meningicoccal and human papillomavirus A Td booster is needed every 10 years.

 *** All individuals who live, work or go to school in the Delaware Health District are considered in district residents. For other individuals who live outside of the health district, an additional $10.00 per visit will be assessed for all adult and childhood immunizations provided. The Health District includes all areas of Delaware County except the cities of Westerville and Dublin and those areas annexed by Columbus.

Childhood Immunizations Required for School

The State of Ohio requires children to be immunized if they are enrolled in any daycare, preschool, Head Start, or in public or private school from kindergarten through 12th grade. The immunizations required include the following:

 
  • DTaP, DPT, DT, or Td vaccine (Diphtheria, Tetanus, Pertussis) 5 doses for kindergarten entry
 
  • Grade 7 - 1 dose of Tdap/TD vaccine prior to school entry
 
  • OPV, IPV (Polio) 4 doses total of either vaccine
 
  • MMR (Measles, Mumps, Rubella) 1st dose at 12 months and 2nd dose by Kindergarten.
 
  • HIB (Haemophilus influenza b) 1-4 doses depending on age when child began series (for Child care/Head start and Pre-Schools) only
 
  • HepB 3 doses for daycare entry and K-11
 
  • Varicella (Var) vaccine - 2 doses for Kindergarten school entry for 2009-2010 school year. If a child has had the chickenpox disease, the kindergarten student does not need the vaccine.

These are general requirements based on a child receiving all of their baby and childhood shots at the recommended times. Religious, medical, and philosophical exemptions to the immunization requirements are accepted by schools if properly documented. Please refer to the immunization schedule, listed on the Health Department's web page, to determine the recommended ages for receiving childhood immunizations. If you have questions about your child's immunization status you can contact one of the registered nurses at the Health Department and your child's record will be reviewed at no cost.
