Mobilizing for Action through Planning and Partnership

The Partnership for Healthy Delaware County has initiated a strategic planning process called "Mobilizing for Action through Planning and Partnership (MAPP)". MAPP is a strategic approach to community health improvement. This planning process was developed by National Association of County and City Health Officials-(NACCHO).

Mobilizing for Action through Planning and Partnerships (MAPP) is a community-wide strategic planning tool for improving community health. Facilitated by your local public health agency (Delaware General Health District-DGHD), this tool will help our community prioritize public health issues and identify resources for addressing them.

Four MAPP Assessments

	Local Public Health System Assessment - Introduction to the 10 Essential Public Health Services PHDC members output from meeting 1 National Public Health Model Standards National Public Health Performance Standard Program (NPHPSP) instrument Version 2 (Instrument used to conduct the 6 hours Local Public Health Assessment in Delaware County). NPHPSP user guide was very useful in planning the elaborate process The Delaware General Health District conducted further assessments in-house with the health department staff. The instruments used for the assessments were: Respondent Information Form (RIF) Agency Contribution Questionnaire Priority Questionnaire Local Public Health System Performance Assessment - Report of Results using the NPHPSP instrument at the 6 hours meeting with the PHDC group. The PHDC group responses were entered into the CDC - NPHPSP website and the analysis of the results was emailed back to us within a week. PHDC group was presented the CDC analysis at a one hour follow-up meeting. The results were discussed and challenges and opportunities were discussed by the community leaders. A summary of the results was prepared. Delaware General Health District conducted further prioritization based results from CDC. The challenges/opportunities worksheet was used to revisit the quadrant distribution (High Priority/High Performance, Low Priority/High Performance, High Priority/Low Performance and Low Priority/Low Performance) with the management staff. The process resulted in distribution change with management consent. The indicators/options that fell into High Priority/Low Performance were selected for prioritization and working on action plans to improve our performance. A ranking process was used to rank the five indicators/options. The results from this assessment will be provided to the Board of health when they will be re-visiting the agency priorities.
