The Partnership for Healthy Delaware County (formerly known as APEX committee) is a community partnership group composed of 55 members with representation from local hospitals, social and governmental agencies, the religious and business communities, philanthropy, academia, local residents and media.

VISION

Delaware County will be a healthier community with individuals who are educated on health issues, who desire good health for themselves and others, and who will share responsibility for optimal community health.

The Mission of this committee is to serve as Delaware County’s lead community-based partnership working to improve residents’ health status by:

Enhancing community organization planning and collaboration on health issues;

Supporting initiatives to build a healthier community;
¨      
Focusing community health resources;
¨      
Increasing access to quality health care; and
¨      
Encouraging the community to be well informed and proactive on health issues.

The Partnership has initiated a strategic planning process called "Mobilizing for Action through Planning and Partnership (MAPP)". MAPP is a strategic approach to community health improvement. This planning process was developed by National Association of County and City Health Officials-(NACCHO)

Mobilizing for Action through Planning and Partnerships (MAPP) is a community-wide strategic planning tool for improving community health. Facilitated by your local public health agency (Delaware General Health District-DGHD), this tool will help our community prioritize public health issues and identify resources for addressing them.

Community ownership is a vital component of MAPP. Because the community’s strengths, needs, and desires drive the process, MAPP is designed to help residents and organizations come together to ensure that resources and needs are aligned to promote health. Community energy and public participation are crucial to the success of this process.

We are actively recruiting additional members for the Partnership for Healthy Delaware County to represent people from different walks of life, geographical areas of the county and people who are interested in improving the health of their community.
