  • Your immunization Record (your doctor can fax records to 740-203-2044)
  • Your insurance Card
  • Your Photo ID

ADULT & CHILDHOOD IMMUNIZATIONS 
Clients with immunization records that need translated into English must make an appointment (walk-ins not accepted). Clients also must provide the immunization record to our clinic at least 2 business days prior to the scheduled appointment. Immunization records can be uploaded here.

All COVID-19 vaccines help protect us from the disease. We do not recommend one over another.

SCREENINGS

  • TB Skin Testing is offered on Tuesdays and Wednesdays, 8:30-11:30am and 1-3:30pm
  • Requires a return to our clinic in 48-72 hours for reading of the test and documentation

  • Pregnancy health education and referrals to other community services are provided by a registered nurse
  • Documentation of a positive pregnancy test is provided to apply for Medicaid and WIC

Accepted Insurances:
Many immunizations are covered by insurance, all can be purchased at a cash pay price, and some may be available on a sliding fee scale (as low as $0.00). In addition, no child that qualifies for the Vaccine for Children program is turned away due to inability to pay.

The following insurances are accepted at the Health District:
• All Medicaid plans
Medicare:
   – Part B vaccines (Flu, COVID, Pneumococcal): Medicare part B and most Medicare Advantage Plans (Anthem HMO accepted. All other HMO plans are not accepted for Part B vaccines).
   – Part D vaccines (RSV, Shingles, Tdap, etc.): Most Medicare Advantage plans and supplemental part D plans. Please call ahead for our staff to confirm coverage.
Private plans: Aetna, Anthem, CareSource Market Place plans, Cigna, Humana, Medical Mutual, Ohio PPO Connect (includes The Ohio State University Health Plan, Ohio Health Choice, Quality Care Partners), UnitedHealthcare network plans.
  – Anthem HMO accepted. All other HMO plans are not accepted.
