You must bring the following to every immunization appointment:

• Shot record.
• If you are a new patient, please upload your record here before arriving to appointment.
• Records that need translated. Please provide us with the record 2 days prior to appointment.
• Insurance Card.
• No Insurance? Cost is $28 the first shot and $15 each additional shot. Sliding fee scale is available for those that qualify. No child that qualifies for the Vaccine for Children program will be turned away due to inability to pay.
• Any custody or guardianship papers.
• Photo ID for adult accompanying child.
